ADP AUTHORIZED VENDOR · CONCORD HEAD OFFICE
Ontario’s Assistive Devices Program (ADP) pays 75% of the ADP-approved price for eligible walkers, rollators, manual wheelchairs, power wheelchairs and mobility scooters. You pay the remaining 25%, and if you receive ODSP, Ontario Works or ACSD, ADP can cover 100%.
Vital Mobility’s Concord head office is an ADP authorized vendor: we submit your paperwork to the government and deliver your equipment within one to two weeks of approval.

Mobility aids you’ll need for 6 months or longer:
Only ADP-listed models qualify, and only the basic equipment you need for daily mobility. Upgrades or extras your therapist doesn’t approve are paid in full by you.
Need a hospital bed? Ask Ontario Health atHome (310-2222) or March of Dimes Canada about funding help. You can also rent a hospital bed from us from $179/month.
You pay the remaining 25% of the ADP-approved price. If you’re on ODSP, or have private health insurance, they may cover that 25% for you. Check with your caseworker or insurer before your order.
If you receive Ontario Works or ACSD, ADP may cover the full cost.

An ADP-registered occupational therapist or physiotherapist assesses your needs and recommends a device. We can help you find an authorizer. If you already have a file with Ontario Health atHome, ask them about a free assessment in your home. A doctor’s note alone isn’t enough.
You have the right to choose your ADP vendor. Tell your therapist you’d like to work with Vital Mobility.

Our team handles the paperwork and submits your application to the government for approval.
ADP reviews applications within about 8 weeks. Once approved, we deliver within one to two weeks. We move quickly so you’re not left waiting.
Once your assessment is complete and we’ve submitted your ADP application, you don’t have to wait for approval. You can buy your device at full price, and once ADP approves, we refund the ADP-funded portion.
Please note: if ADP doesn’t approve the application, the purchase stays at full price. If you buy before your assessment and application are completed, ADP won’t contribute.
No. For mobility aids, ADP requires an assessment by an ADP-registered occupational therapist or physiotherapist.
We can help you find one. If you have a file with Ontario Health atHome, they can arrange a free in-home assessment. You can also email adp@ontario.ca for a list of registered therapists in your area.
Assessments through Ontario Health atHome or a hospital are usually free. Private therapists may charge a fee, which ADP doesn’t cover.

Yes, non-folding power scooters, for people who meet ADP’s criteria as assessed by an authorizer. Folding and portable models aren’t covered.

Yes. It’s your choice. Just tell your therapist which vendor you’d like.
